Your username
Your email address is needed to let us know who
this request is coming from. Requests for permissions
to use Library data need to come from authorized
staff members.[Return

Account owner's
username
The username will be the name used by this person
to login to the Data Warehouse. This username should match
the name used for Athena and email access. New
staff members will need to have their Athena account
set up before requesting a Domain account. For
temporary staff who do not have Athena accounts,
we will attempt to create a name unlikely to be
used by an Athena user. [Return

Location
Location is used primarily for our records. If
a staff member works in 2 locations, choose one
for the purpose of this form. [Return

Account changes
New account: To your knowledge, this person does
not already have an account for the Data Warehouse.
New password: This person has forgotten their
password and needs a new temporary password to
be assigned.
Modify permissions: This person already has an
account, but needs additional, or reduced, access.
Delete: This person will no longer be working
for the Libraries.
