Domain Account Request: MITLIBRARIES Domain accounts for staff members

Use this form to send information to Systems so we can properly configure MITLIBRARIES Domain accounts for staff members.

  • Add new users
    (Users must know their Athena user name before asking for a new Library LAN account)
  • Delete users who have left the Libraries
  • Change users' information

Account Request Form User's Guide

Your username

Your email address is needed to let us know who this request is coming from. Requests for permissions on the Libraries domain need to come from authorized staff members.[Return to form]

Account owner's username

The username will be the name used by this person to login to Windows. This username should match the name used for Athena and email access. New staff members will need to have their Athena account set up before requesting a Domain account. For temporary staff who do not have Athena accounts, we will attempt to create a name unlikely to be used by an Athena user. [Return to form]

Account owner's title

The staff person's job title. [Return to form]

Location

Location is used primarily for our records. If a staff member works in 2 locations, choose one for the purpose of this form. [Return to form]

Account changes

New account: To your knowledge, this person does not already have an account on the Libraries domain.

New password: This person has forgotten their password and needs a new temporary password to be assigned.

Modify permissions: This person already has an account, but needs additional, or reduced, access.

Keep account: Staff accounts are deleted at the end of the month in which that staff member leaves the Libraries. Student accounts are deleted at the end of the school year. Explain why the account should be kept.

Delete: This person will no longer be working for the Libraries, or will be leaving for a period of time (e.g. a student leaving for the summer). The account should be removed. [Return to form]

None- This is provided so that permissions may be removed, also so that you can clear your selection if you chose the wrong permissions by accident. It is not necessary to choose None if no permissions are needed.

 

Domain user- This is a normal domain account. Most staff members will have this type of account.

Supervisors or LTE's may request.

Student or Casual- This account is for limited term of employment accounts. A supervisor of the student/casual worker will be notified once a year that this account may be deleted.
